I think the best question is why does your doctor want you to avoid malaria areas, is it the malaria itself or the preventative medication.
If you want an elephant, you could hunt the winter months in a lot of those places you mentioned and not have to worry about Malaria.
If you are ok with just buff, I would just stay within South Africa and not worry about it. Just find a big enough concession and enjoy.

I believe some of you misinterpreted my question. I never asked for a risk analysis for me going to Africa or other traveling advises, I use my doctor for that. (Who by the way have spent a lot of time working in sub-tropical Africa and he is a cardiac surgeon). All I asked for is if there are buffalo / elephant hunts available outside fenced area and outside the malaria zone. I now understand that most or all outfitters in RSA offering buffalo hunts have fenced areas, large or small.
