Electrical tape over muzzle I’ve seen this with ice in muzzle do same thing

Was just at the local gun range a couple days ago and one of the staff showed me an OU that had blown up like that. Bottom barrel completely sheared off, it was determined a previous shot malfunctioned and the wad/shot was still stuck in barrel when he tried again.
Luckily no one was hurt.
