From what I understand there are no COVID requirements for leaving the US/entering SA, correct? Are there any requirements for leavig SA/entering the US?

No there is not as of today. You should bring a copy of your vaccinations just in case anyone may question you from the airlines to customs.

Do they require the COVID vaccine? I have not taken it and never will

I have not been vaxxed and have been once to RSA since covid and did not have problems.
