4457 question

Is the 4457 form universal? In other words, if I go to airport x to get one can I use it if I fly out of airport y?

Its universal for U.S. citizens regardless of what country/airport you are in. I get mine and fill it out at a Border and Customs office and fill it out there. Then just take it with me to the airport with all of the other paper work.

You can print it out online too. I had to show the agent where all the serial numbers were? He worked there for 10 years and never saw one before?
