SAPS 520 Form - Postal Code?

I'm filling out the SAPS520 form for our upcoming trip. Trying to decide how to fit my 5-digit zip code into the 4 boxes for postal code on the form. How has everyone else done this?

Yes, and just make it fit. The first time I did the SAPS form I had to call Henry and get his guidance. That was one of my questions.

Just put starting in the left most box and put the last two digits to the right outside the boxes. They will understand.

Lori at Travel Express has a example form filled out for you to look at if there is any questions on filling one out.

I just left mine blank like it shows in the example.
