Same here. Half our group filled out the form, and the other half ignored it. Nothing was ever said about it by anyone. Total non-event. I'm embarrassed with the amount of time I spent figuring it out.

Just got back from traveling through Jbrg to Port Elizabeth and back via East London to Jburg to Atlanta and never was asked about the form, no kiosks and no one seems to know the form even exists. I did fill out the form before departure, it takes a few minutes but does not ask for any information they don’t have already from your passport when you enter the country.

This was my experience as well. Most of our group completed the form. Others did nothing and everything was fine for both. Nothing was ever mentioned and it was much ado about nothing.
